To be placed on our waiting list, we request that all prospective new owners fill out this questionnaire.  This provides you with information and helps us determine what puppy is best for you and how we can best advise you on introducing a new puppy to your household.  This information is considered confidential and will not be shared.

Chihuahuas are indoor dogs who may not be willing to go outside to do their "business" and may need to have an alternative available, such as wee-wee pads or newspaper. Are you prepared to clean up after your dog for the rest of its life? ____Yes ____No

Are you aware that you need to keep your puppy off public ground and away from any potentially unvaccinated animals until all three puppy shots have been given? ____Yes ____ No

Second and third puppy shots are required at 3 and 4 months. At about 6 months puppy teeth that do not come out on their own may need to be pulled, the best time for this is when puppies are spayed or neutered.
