questionnaire. This provides you with information and helps us
determine what puppy is best for you and how we can best advise you on
introducing a new puppy to your household. This information is
considered confidential and will not be shared.

Second and third puppy
shots are required at 3 and 4 months. At about 6 months puppy teeth that do not
come out on their own may need to be pulled, the best time for this is when
puppies are spayed or neutered. As adults, annual shots and checkups are
